• Patient records: booking details, clinical notes, prescriptions, referrals
• Financial records: invoices, medical aid claims, payments, tax records
• Employment records for our staff
• Supplier and laboratory records

How to request a record

Send a request on Form 2 (available from the Regulator's website or from us) to the contact details above. Prescribed fees may apply. We answer within 30 days. Your own patient record is usually available sooner: just ask.
